If you transferred your apps from your old phone to your new phone, you should see the same devices in your Google Nest "home/structure" in the Google Home app as long as you are logging on with the same Google Account you were previously using and accessing the same Google Nest "home/structure".

If you did NOT transfer your apps, if you install the Google Home app on your new phone, you should see the same devices in your Google Nest "home/structure" in the Google Home app as long as you are logging on with the same Google Account you were previously using and accessing the same Google Nest "home/structure".

(You may only have one Google Nest "home/structure". We have only one, for the Google Nest devices in our one home.)
