topic Constant HW ON signal in Nest Thermostats

I have a Nest 3rd Gen Learning thermostat. With both CH and HW selected OFF and the boiler switched OFF, something occasionally causes the Nest to send an open signal to the HW motorised valve. The valve doesn't like a permanent ON signal. It starts buzzing and overheating. It seems to be associated with this yellow icon on the screen for which I have failed to find any decode.

I think it may be associated with a power cut or an interruption to the wifi signal.

Anyone know what's going on here and what I can do about it?
