I have my own cable modem (not a modem/router combo). I have Astound(RCN) ISP with 250/20 mb/s and see at least those speeds if I plug a computer directly into the back of the modem. So it's obvious that everything from the outside world into my house all the way to the ethernet jack at the back of the modem is great. Plugged into that jack I have a Nest main router (Model H2D 14150.376.32) and 3 access points. I have no automations and no IPv6. Roughly once per day, the WiFi (or wired) speeds from that router get really slow (e.g. 3mb/s upload).
If I power cycle the main router (or just press "Test Network" on the Google Home app) the WiFi speed instantly jumps back up to 150/15 (and the wired speed is 300/20).

Sound like you are running double NAT, where the ISP equipment is not in bridge/bypass mode? This is known to cause performance issues with Nest Wifi and should be avoided. More on that:
