google assistent works on TV but is displayed offline in the google home app

A few questions: do you have a Chromecast device or does your TV have a built in Chromecast? If not, does it have a built-in Google Assistant? Also, did you use the Google Home app in setting it up?

Make sure your TV and phone are connected on the same network. Try uninstalling and reinstalling your Google Home app.

Could you try rebooting your TV and router for a minute or two to refresh the connection. Also, try uninstalling and reinstalling your Google Home app.
