Nest Thermostat fan controls

I just installed my new Nest Thermostat. I followed all the instructions but am having a few problems. 

First, I don't see the fan on/off icon on the face when I tap the side. How am I to be able to turn it on or off if there is no icon?

Next, it seems like my fan is running constantly. I have it in cooling mode and I am not sure the fan has ever shut off.

Any help would be greatly appreciated.

@pastormrlyon, since you followed all of the instructions of the installation, then that means you have a picture of your old thermostat and the wires that were attached to it. Do us a favor and upload that picture. While you’re at it, why not upload the picture of your nest backplate So we can see how the wires are attached in that one.
