what do I need to have so I can hear my nest doorbell when someone is at the door inside my house. Not just on my phone. For example I am downstairs but my phone is upstairs

Do you have a wired doorbell connected to an indoor doorbell chime? If so, you can turn on the "Indoor chime" option.
Whether your doorbell is wired or not, you can turn on the "Visitor announcements" setting and get announcements sent to Nest Hubs, Nest Minis, and Nest speakers:

@MplsCustomerthat's is actually not entirely true - I have setup my Nest Dorbell with the rest of my smarthome solution (Google Home and Google Nest2) - The first 2-3 rings on the doorbell goes to all my units - but after that, only my phone recieves the notifications. Thus rendering my entire Nest Doorbell useless. I have already missed out on several visitors, delivery guys and even my family... !
Please fix the issue - or at least reach out to me.

I'm just another Google Nest customer, but are you talking about visitor announcements and doorbell chimes not occurring when the doorbell button is pressed repeatedly in close sequence? I think there are issues with Google Nest doorbells not sending either "Visitor announcements" or "Indoor chimes" on second or subsequent attempts when the doorbell button is pressed repeatedly. Our doorbells don't get rung repeatedly very often, but my recollection is that subsequent repeated rings are not transmitted, and I recall seeing past posts in this forum about this.
Perhaps it is a technical limitation due to the same "cooldown" period that causes the camera to not send repeated alerts in sequence, as noted by Google Nest in this post:
