I have a nest cam w battery 3 months old. Have provided cam charging cord and power supply plugged into functioning outlet. Camera has shutoff due to dead battery. Have tested the outlet, it operates correctly. Why is my battery not staying charged ?

Are you having problems charging your camera? If so, did you dismount your camera to charge it, and is the charging cord plugged in securely in the bottom of the camera with the charging cord feeding towards the FRONT of the camera?
Or are you using the 1-meter charging cord to continuously power the camera, with it plugged in all the time? If so, have you had the camera plugged in since you first installed it? Some customers in this forum have reported that if they plugged in their camera AFTER it was installed, they had to remove the camera and do a factory reset in order for it to properly recognize it was plugged in.
Also, it's not clear to me whether you can use the 1-meter charging cord to power the camera, rather than the optional 5-meter or 10-meter power cables. Google Nest would have to answer that. We've had a battery camera for over 2 years, plugged in from the start with the 10-meter optional power cable, and it's been "Live" and charged the whole time.

I’m using the 5 meter cord supplied with camera and it’s been plugged since first opened. I have plugged and unplugged cord to make sure connection is good. Camera power light turns on but on app shows battery is empty.

The camera is supplied with a 1-meter charging cord ("1 charge cable, 3.3 Ft. [1m]"); I'm not sure that cord can be used to power the camera, as opposed to charging the battery. The optional 5-meter and 10-meter power cables are purchased separately (https://store.google.com/us/product/nest_cam_weatherproof_cable?hl=en-US).
If we go to Settings | Battery for our camera, it always shows as "Plugged in" with an infinity symbol.
