When the Chromecast with Google TV (CCwGTV) has its own audio options set to stereo, the audio and video are in sync, regardless of whether the TV's Game Mode setting is on or off. However, when the CCwGTV is set to surround sound and the TV's Game Mode setting is on, the audio lags behind the video. When Game mode is off, audio & video are back in sync.

Hey there GrillMouster,
I apologize about the late response, thank you for your patience on this! Were you able to fix that audio sync issue? If not, I'd be happy to help out, let me just ask you some questions about what's happening:
I look forward to your response, and if you have any other questions or concerns for me, please let me know!
Best regards,
Jeran

When did this issue start happening? I noticed it as soon as I received and set up the Chromecast with Google TV on November 30th 2021.
What model of TV do you have? I have a TCL Roku TV (model 65R615).
What is the firmware version of the Chromecast? It's happened on every version of the firmware, including the current version, QTS1.210311.############8, with the Oct 5, 2021 security patch and kernel 4.9.180.
Are you using speakers or a soundbar? I use a Vizio SB3651-E6 soundbar connected to the TV by HDMI ARC. The same audio delay happens when I connect the soundbar using optical connection. The audio delay also happens even with the TV's built-in speakers when I disconnect the soundbar.
What troubleshooting steps have you already tried? Rebooted everything, factory reset, tested all 3 of my other Chromecast with Google TVs with the same results. Tried with different HDMI ports, different cables, with and without soundbar. The problem keeps happening with game mode enabled on the TV.
