I’m having issues completing the warranty form for a defective base plate. It keeps asking me to refresh the page and sorry we’re having problems with the form. How do I resolve this?

Oh no, sorry about that. Have you tried the form on another browser? If yes but to no avail, what Nest thermostat are you pertaining to? What happened to the base? Also, when and where was it bought? I'll be glad to check on your warranty options.
