Can someone look at my 2-wire pic for my insert heater and let me know if the Nest Thermostat will support it without a C wire?

Where is the pic?
Is this a fireplace insert?
If yes they are usually millivolt systems that don't have a 24v transformer.
The only way to get it to work is with a plug in 24v ac wall transformer connected to R and C.

yes, it is a gas fireplace insert (15yrs old or so)

You need one of these. https://www.amazon.com/dp/B085TLYBFX
You put one transformer wire into C
The other transformer wire together in the same terminal with the R wire on the Nest.
Then put your W wire in W.
This will provide constant 24V AC to keep Nest powered up and charged.
