I got an email from googlenest @ google dot com saying my router has an issue and can be replaced for free.
However, the link for “this form" doesn't work, it goes to this broken link I’ve attached a picture of… can someone please help? I just noticed the rebooting issue a few days ago while working from home.

When I try to submit the page I get an error saying that it could not be submitted, refresh the page and re-submit. I've tried this 5 times and over a couple hours and it still doesn't work.
