Hi andersW,
Thanks for reaching out. Could you please plug in the Chromecast with Google TV (4K) directly to the HDMI port on your TV and check if the same issue persists? Rebooting your Chromecast and your AV receiver might also help.
If the issue persists, do a factory reset on your Chromecast with Google TV. Here's a guide: How to factory reset your Chromecast.
Thanks,
Jennifer

Hi, thanks for checking in, @JenniferV , but as I didn't get a response sooner I found the Google TV community and did a cross-post there and got some feedback.
I've answered that question there: https://support.google.com/googletv/thread/196898854?hl=en&msgid=197057191
To summarize, I did a factory reset already, and also re-applied the latest firmware on my receiver and it is better, but not 100%.
Netflix still loses the front-channel sound (speech basically) now and again, and in periods I get some "muffled" sound when using Dolby Digital 5:1, or Dolby Digital Plus 5:1, from the receiver.
It is not happening (that I've noticed) from HBO or Prime App so seems to be isolated to Netflix App only.

Hello andersW,
Thanks for the update. If the issue is isolated with Netflix, I'd suggest you reach out to Netflix support for additional help.
Best,
Jennifer
