Nest replacement email

I received an email from google that there is an issue with my thermostat and google has offered to replace it for free.  How do I process the replacement?

Thanks for reaching out to us. Could you share here the screenshot of the email that was sent to you? Also, answer the following:

• C-wire power source (C-wire to HVAC system, add-a-wire, or AC adapter)
• Photo of wiring to the backplate
• Did you add a C-wire or change your wiring after you paired?
• Heating, ventilation, and air conditioning (HVAC) system type
• When was the device first paired?
• Power reading from Tech Info (VIN, IIN, PS-*)

I have not made any changes to my Nest wiring and has been working fine for the past year or so.  My nest did have an error stating it was not working properly this am and then also noticed I received this email below -
