Nest thermostats heating wont stop

The nest turns heat. When the temperature set point is reached, the thermostat won't turn off the power. After I removed the thermostat from the back plate, the heating system soon shuts down. But the AC comes on when I put the thermostat back to the plate. I checked the wire is connected correctly, so don't know what is the issue in here. Please help.

• What type of Nest Thermostat do you have? 
• Have you tried to do a Factory reset on your thermostat? 
• Have you done any troubleshooting on your Nest thermostat? 
• What type of HVAC system do you have?
