I have a second generation thermostat. On a few occasions recently, we have found that the heat in the house is very high and have looked at the thermostat which shows that it is set to Manual mode and so the boiler is still on when it should have switched off. The thermostat offers the possibility of changing back to automatic which I select and things go back to normal. Nobody has done anything in the house to put it in Manual mode.
I spoke to Adrian at Nest about this and he told me that it is impossible for this to happen! The heat link button has to be pressed for it to happen. However, there are only two adults in the house and neither pressed the button - why would they and then ring up and complain to Nest about the result!
In spite of Adrian's assurances (and his unwillingness to accept that anything is wrong with the Nest product) I see that two other people have complained on this site about the same problem in the last few months including a person with a G3 thermostat:
Unfortunately, neither issue was resolved - partly due to the original posters not pursuing the issue. I suppose that eventually Google will admit there's a problem and fix it.
Has anyone else had the problem? Is anyone else at Google willing to help?

This has just happened to us- the outside temperature is 36 degrees celcius but our Nest turned itself on to 29 degrees and manual.

Yep. Just found my thermostat set to manual mode. Half of the day on. Oh my bill. Looking for alternatives, this is pretty bad.
