When choosing 'Auto (CEC)' under 'Settings -> Remotes and accessories -> Set up remote buttons -> Volume control', sound is no longer output from the Google TV Streamer. In this mode, the Google TV Streamer controls the TV's volume just fine (via CEC), but appears to set its own volume to 0.
When choosing the setting 'TV (IR)' to control the TV volume via the Google TV Streamer remote using IR everything works as expected and sound is output.
When choosing the setting 'Google TV Streamer', volume control works and sound is output. However, which volume Google TV is set to internally depends on which setting was chosen previously.
When switching from 'TV (IR)' to 'Google TV Streamer', Google TV's volume is at 15 (or whichever volume was previously chosen by the user in this mode). When switching from 'Auto (CEC)' to 'Google TV Streamer', Google TV's volume is at 0 and requires turning back up.
I have noticed there are at least 2 other posts by separate people regarding this issue, so it would appear to be a bug with Google TV. While it isn't a big problem for me due to the IR control working fine, it is still an annoying bug that limits functionality such as controlling volume through the app on another device.
Though I doubt this issue is specific to which TV model the user has, my TV is a Hisense 43E6NTUK.
