I have had my google home mini for a few years now. I no longer have access to the email it was set up under or the phone that I had the app on. I recently got a new modem and they changed my wifi. How do I reconnect my device?

You can try performing a "factory reset" on your Nest Mini:

After that, you can try reinstalling it in the Google Home app.
