Nest N260 message still appearing after installing Nest power connector

Hello, I've installed Nest and power connector as I have two wire heating-only thermostat system.  Getting N260 warnings and emails about nest getting insufficient power.    Reset the thermostat and ran power test with these results:

Vin:26.6 V

lin: 42(w)

PS: w-42

Looks ok, but still getting the 260 error and it has me worried its not working correctly and will drain battery.

How did you wire the Nest Power Connector into the system?

Are you using an external 24vac transformer? If you are, did you wire it like this?

HI Patrick,  no I'm not using a transformer.  I'm wired just like the "heat only" wiring diagram in the app where there is a COM terminal in my thermostat distribution box next to my boiler.  I can send you a picture if you can tell me how to upload it.
