5th chromecast audio can't be detected by home

This is a bizarre one.  

 
1.  I can cast to the chromecast audio by going into the device and casting directly.
2. I can cast to the audio from google play music by playing a song and clicking the cast button
3.  If I click the "play music" button under the audio device in home, it states it could not play music
4.  If I ask google assistant to play music on that device using its name, google assistant can't find the device - it basically doesn't exist
5.  If I ask google assistant to play music in the room the device is in, it tells me to select the device, but then can't find any devices.
6.  If I click play music or ask assistant to play music on any other chromecast audio (4 others in the house), it will play music.  If I go to home->media and add the chromecast audio to the playing group, it succeeds and starts playing music
7.  If I add the failing device to a speaker group and try play music/assistant, it fails.
 
Have factory reset it twice.  Firmware is the same as the other 4 devices.  This seems like an issue with Google home

Hello, thanks for taking the time to post your issue.
How many concurrent wifi connections does your wifi router and Internet Service Provider's network box or Gateway support? Such as, cell phones, Chromecast devices, smart home devices, printer, wifi mouse, smart light bulbs, laptop, thermostat, etc
Is the 5th Chromecast device on the SAME Wifi channel as other devices?
